Consent to Communicate via Nonsecure Means

We take seriously the issue of protecting the personal health & financial information entrusted to us, and wish to inform you that this web contact form does not meet the standards of HIPAA security. While most people feel comfortable communicating with us in this way, we first request your permission to communicate with you by nonsecure means.

1.    Technical experts often describe email & messaging by way of web forms such as this as being like a postcard, in that it can be viewed by all hands it passes through. Before crafting your message, please consider risks of emails being viewed by various engineers, administrators, and bad actors as it passes through the internet. Use this to guide what you choose to share here.

2.    Think about where you read and write emails, and what devices you do that on. Consider who can see you reading and writing emails in these places, and who can access the devices you use to read and write emails. Would there be any negative consequences to any of those people reading or glancing at emails exchanged with SOUND Life Recovery? Are there certain kinds of email contents that you would feel safe letting these people see and other kinds of contents you would not feel safe letting them see? Use this to guide what you choose to share here.

3.    Think about which email address(es) you might use. Who has access to each address? If you use a work email address, know that your employer may legally view all the emails you send or receive with that address. Be aware that engineers and administrators at your email service provider may be able to view your emails.

4.    How quickly do you normally receive replies from others via email? Do you expect replies more quickly than we may respond? Can you see any negative consequences occurring if SOUND Life Recovery does not or cannot reply to an email as quickly as others in your life typically do?

Currently, SOUND Life Recovery uses an email service called Hushmail for Healthcare, a HIPAA secure service for healthcare providers. Not all messages through our email system are HIPAA secure, and this security measure requires a setting change for each individual email recipient. If you prefer more security than this web form offers, you can choose to phone us to discuss your needs or to request we open a secure, password protected email channel for you. Should we change service providers in the future, our new email service providers will also have HIPAA security standards in place.

By agreeing to this document by selecting the consent checkbox, you are stating:

  • you have read this document and considered the risks, including but not limited to issues of confidentiality in transmitting protected health information by non-secure means.

  • you understand you are not required to sign this agreement in order to contact SOUND Life Recovery, and that you can call to communicate on the telephone or to request a secure email channel be opened.

  • you are stating your choice and your agreement to communicate with SOUND Life Recovery in a manner that does not meet HIPAA security standards.

  • you will take care to communicate in your message only what you are comfortable sharing, with this in mind.

You may contact us to discuss your care or request a secure email channel by calling 360-240-9288.
